Plastibell / Circumplast

This technique is the best method for baby boy circumcision. After anaesthesia is applied, a small plastic ring is placed under the foreskin and tied off with a thread. This thread cuts off blood flow to the foreskin being removed, causing it to naturally detach within 2-3 days. No stitches are needed.
Once it detaches, the wound stays completely DRY. Which is why healing is so fast. Some wounds heal in as little as 3 days.
On top of that, the Plastibell technique gives a neater, more cosmetically refined result. Laser

This is the traditional technique, most commonly used for circumcising children, though it can also be adapted for babies with a different approach. After anaesthesia, the foreskin is clamped and cut using diathermy (laser). The wound is then stitched around the penis, and the stitches fall out on their own.

"Baby circumcision is painful, poor thing."

Baby circumcision actually heals faster and is less painful compared to older children, because pain nerves aren't fully developed yet. Modern anaesthesia also makes the procedure more comfortable.

"Better to wait until my child is older before circumcising."

In reality, waiting longer increases the risk of trauma and fear in the child. Healing time is also longer. Infant circumcision is usually easier and heals faster because of the higher number of healing cells, faster cell regeneration rate, and babies being far less active.

The modern procedure uses a safe local anaesthetic, so the baby only feels slight discomfort during the injection of the anaesthetic. After that, the baby feels nothing at all throughout the procedure. In addition, circumcision at this age is generally less painful because the nerves are not yet fully developed.

Usually from as early as 1 week old up to 2 years old.

The Plastibell/Circumplast ring will fall off on its own within 5–7 days. For most babies — about 95% of those circumcised at Klinik Tuah — no follow-up visit is needed, thanks to very smooth healing.

At Klinik Tuah, we also welcome many parents from abroad — Brunei, Thailand, Singapore, Indonesia, Japan, and more. They usually return to their home country after day 2, and stay in touch with us via WhatsApp to make sure healing is going well. Usually 5–7 days.

For Plastibell, the fastest case we’ve had — the ring detached after just 3 days, with the wound already dry. That said, healing time varies from baby to baby, depending on age, genetics, individual healing rate, activity level, and other factors.

Baby circumcision is the easiest to care for compared to any other age group.

Day 1–2:

  • Give the baby pain relief syrup according to the dosage and schedule set by the doctor
  • Avoid soaking the baby in a pool/bathtub for the first 24 hours
  • Change diapers more frequently than usual — don’t leave a wet diaper on for too long
  • Diapers can be worn as normal, or slightly loosened if preferred
  • After urinating or passing stool, rinse with water/wet wipes as usual — but be sure to dry the area afterward with tissue or a fan

Day 2–3

  • The baby usually looks comfortable by this point — much like before the circumcision

Day 5–7

  • This is usually when the Plastibell ring falls off

Overall, baby circumcision aftercare is very simple.

  • Urination — There should be no issues urinating. If your baby has difficulty urinating, contact the clinic or doctor. Rinse with water as usual, or wet wipes can also be used — just be sure to dry the area afterward with a towel, tissue, or portable fan
  • Stool — If the baby passes stool or urine, change the diaper immediately — don’t leave it too long, to avoid risk of infection
  • Looseness — Use loose diapers and loose clothing until the wound heals
  • Bathing — Baths can continue as normal, just be careful around the circumcised area to avoid pain or dislodging the Plastibell ring
  • Breast milk — Continue breastfeeding regularly after circumcision. Breast milk has natural pain-relieving properties and can act as a natural painkiller, helping ease discomfort after the procedure.
  • Mentally — Be mentally prepared. Ideally, before stepping into the clinic, parents should already understand the circumcision process for their baby, and know that this process is painless 🙂
  • Milk — It’s best to feed your baby before coming to the clinic, to help keep them from being cranky
  • Pacifier — Bring a pacifier or teat. If the baby feels uncomfortable being held during the procedure, the pacifier can help calm them
  • Diapers — Bring extra diapers to change after the procedure
  • Loose clothing — Dress your baby in loose clothes and pants
  •  
  • Once you arrive at the clinic, the doctor will explain the circumcision process in detail. Feel free to ask the doctor anything you’re unsure about.

    • The baby will be laid on the procedure table for the circumcision
    • Anaesthetic will be administered using the finest needle at the base of the penis. The baby may cry at this point due to the discomfort
    • A clinic assistant will hold the baby’s legs in a “frog position” throughout the procedure
    • Some babies cry — but not because the circumcision itself hurts, rather because they’re uncomfortable having their legs held and being laid down. After the anaesthetic is given, the doctor will test its effectiveness to ensure the baby feels no pain during the procedure
    • Parents should stay calm — the doctor will carry out the procedure with full care and responsibility
    • If you don’t feel comfortable watching the procedure, you’re welcome to wait outside while it’s carried out
    • Once done, a fresh diaper will be put on, slightly looser than usual
    • At this point, the baby is usually calm and no longer crying, as the local anaesthetic is still in effect. Pain relief syrup will be given and should continue to be administered so its effect kicks in as the anaesthetic wears off. Make sure the syrup is given regularly, every 5–6 hours, to keep the baby comfortable until fully healed
    • Parents can go home with peace of mind, knowing their baby was circumcised safely
    • For Plastibell, the ring will detach on its own within 2–7 days. If it hasn’t detached after 7 days, don’t worry — just contact us or come in to the clinic
    • For Laser circumcision, the stitches will dissolve on their own within 7 days — no need to have them removed at the clinic
    • During healing, you may sometimes notice swelling that looks like a small water balloon around the area — this is simply swelling from the anaesthetic. No need to worry, it isn’t painful and will fully disappear within 2 weeks
    • After a few days, you may also notice what looks like “pus” or a crusty yellowish coating on the tip of the penis — this too is nothing to worry about, it’s just normal fluid. It will dry up and come off on its own after a few days

Plastibell usually falls off within 5–7 days. If it hasn’t detached, it can be left to fall off naturally up to day 10. If it still hasn’t come off by then, WhatsApp or call us — this is extremely rare. Usually, in cases like this, the ring has actually already detached internally, but dried fluid is causing it to appear stuck.

Every procedure, no matter how simple, carries some risk of complications, though this is very rare.

Possible risks include:

  1. Bleeding — Bleeding is the most common complication, whether for adult, child, or baby circumcision. However, it’s not dangerous if treated promptly. This minor bleeding can sometimes happen suddenly at home if the wound is bumped with moderate impact. If bleeding occurs, take a tissue or towel and apply firm pressure to the area for 5 minutes — this usually stops the bleeding. If it doesn’t stop, contact your doctor for help.
  2. Infection — Infection of the area is rare but can still occur. Signs include warmth in the area and clearly visible redness. Contact your doctor for antibiotic treatment if this happens.
  3. Plastibell detaching too early — If the Plastibell detaches prematurely (within 1 day of the procedure), inform your doctor immediately for further care. This is very rare.

The risk of complications remains, even though our doctors are highly experienced, having performed thousands of circumcisions. If it happens, don’t panic!
